How does a neurofeedback session work? An ADHD treatment as example.

Neurofeedback started as early as the 1960’s when researcher Joe Kamiya experimented with the Alpha activity (8-12 Hz) in the human brain. The results, which were very promising, inspired many other new experiments. Nowadays the fact that humans can learn to alter their brain function, by using an EEG, is virtually undisputed.

Details Matter:
The Meds Don't Work Long Enough: The Duration of Effectiveness [DOE] is not adequate: Each stimulant drug, each family of stimulant drugs has its own expected DOE. This DOE is not discussed in the literature as a measuring tool, but it's common knowledge with any who watches carefully for drug action and duration throughout the day.

All stimulant medications have an expected, less than 24 hr duration. Duration is an obvious point but is overlooked so often as an essential tool. If meds last less than 24 hr, then how long should each last? Dialing in the particular duration is necessary to get the best action out of each specific med. [AMP=Amphetamine Family, MPH=Methylphenidate Family]

Vyvanse [AMP] and Daytrana [MPH patch] both win the DOE race with 12-14 hr expected as optimum.

Adderall XR [AMP] is next with 8 hr DOE -- sometimes can get 10, but often more DOE is too much - out the top of the Therapeutic Window: w

Concerta [MPH] and Focalin [MPH] both run 8 hr if dialed in effectively - some studies show longer, my experience: more than 10 hr with either of these is too much -- see other articles on the 'Top of the Window.'

Metadate CR [MPH] and Ritalin LA [MPH] are both right at 8 hr, rarely longer, with side effects, if pushing the dose higher

The Immediate Release Stimulants, the tablets with no extended time
expected, last only a portion of the day with Adderall IR [AMP]
[Immediate Release Tabs] lasting about max 5 hr, Ritalin IR [MPH] is 4-hour max duration.

Why are clients with ADHD experiencing such different symptoms? ADHD symptoms can include anxiety and depression, as well as other disorders. It is important to evaluate clients’ physiology, family system, nutrition, exercise and other factors to rule out any possible causes.

This article suggests that ADHD can be associated with multiple etiologies. Therefore, ADHD treatment should always be tailored to the individual patient. 75% Children diagnosed with ADHD will experience permanent symptoms throughout adulthood.

The majority of reports about neurofeedback for ADHD follow a standard protocol. This could be the same for everyone or it could be adapted for each patient’s Q EEG analysis. There is a more personal approach called QEEG informed, or Q EEG-guided neurofeedback. This type of neurofeedback does not allow for the selection of a protocol and application to all participants. Instead, the subjects are given a customized neurofeedback protocol after a QEEG analysis. This form of neurofeedback is used successfully in schizophrenia, obsessive-compulsive disorder (OCD), migraine, dementia, and children with learning disabilities.

Only two studies have been published that used Q EEG-informed neurofeedback in ADHD patients. It is therefore not possible to do a meta analysis of the effects of this type neurofeedback on ADHD. Both published studies have shown a positive impact of neurofeedback.

There are 18 schemas or core beliefs that have been identified, but this video covers 3 that have been identified to be common in adults who have ADHD, even if you were never diagnosed as a child.

Schema 1- Feeling defective
You feel inadequate at all levels. This can make you hypersensitive to criticism and rejection. You can also feel insecure around people and always be in a state of comparing yourself to others.

Schema 2 - A basic sense of failure.
Essentially you feel you haven’t met your potential. With this schema people often feel inept, stupid, lacking talent or just doomed to be less successful than everyone else. This can cause you to give up easily on things or not even try something because after all, you’re not going to be good at it.

Schema 3 - Insufficient self-control
You can’t rely on yourself to accomplish the things you need to. You can’t tolerate the frustration that comes with waiting for a desired outcome. Also through this schema you avoid certain responsibilities or duties that are uncomfortable even if it means missing out on something that could advance you.

Over the years you can develop a cycle of negative experiences that lead to a negative appraisal of yourself. This leads to maladaptive coping strategies like procrastinating, or avoiding things altogether. This creates negative emotions like depression, anxiety, guilt, anger and frustration.

Then these coping strategies and negative emotions confirm your core beliefs that you’re defective, you're a failure and you have insufficient self-control.

What can you do about it?
One goal is to change your core beliefs about yourself because those beliefs perpetuate the dysfunctional behavior like avoidance and procrastination which creates more problems for you. This is best done with the help of a therapist who is trained in Schema Therapy.

A second goal is to change your coping strategies in a way that modifies your schema or core beliefs. The new feedback that you get from others and yourself becomes more positive. Your schema is formed from your experiences and how you respond to input you get externally. Therefore, consistent positive feedback helps to change your schema.

Why do clients with ADHD have such vastly different symptoms? Anxiety and Depression, amongst other disorders, can often exhibit ADHD symptoms. When evaluating clients, its important to look at their physiology, family systems, nutrition, exercise, to look at all possible causes.

The article suggests that since ADHD is associated with different etiologies, ADHD treatment should be personalized, whether pharmacological or non-pharmacological, to reach an optimum effect in the majority of patients (5-7% children/1-7.3% in adults) 75% of children diagnosed will face permanence of symptoms into adulthood.

Most of the reports on the use of neurofeedback for ADHD use a standardized protocol, either equal for all participants or, adapted to each patient’s post Q EEG analysis. However there is another more personalized approach known as QEEG informed or Q EEG guided neurofeedback. In this variant of neurofeedback, rather than selecting a particular protocol and applying it to all the participants, subjects receive a neurofeedback protocol selected specifically for them after a QEEG analysis. This type of neurofeedback has successfully been used in schizophrenia, obsessive compulsive disorder, migraine, dementia, and with learning disabled children.

There are so far only two studies applying Q EEG informed neurofeedback in ADHD patients, so it is not yet possible to perform a meta analysis on the effects of this type of neurofeedback on ADHD. However, a positive effect of neurofeedback has been reported in both published studies.
