Working memory is often referred to as an active mental workspace. It’s a cognitive system that allows us to hold and manipulate information online for a few seconds during problem-solving or other cognitive activities. And it’s his internal representation in working memory that we use to guide our decision-making, so that behaviour and actions are not dominated by immediate sensory cues in the environment. By contrast, long-term memory is often referred to as a filing system for a more stable or longer-term storage of information. And typically cues in the environment or in working memory trigger the need to rapidly retrieve information from long-term memory and bring it into working memory along with other types of information. Conversely, information in working memory may be reorganized and encoded back into long-term memory. Here’s what might happen when a teacher gives instructions out of order and they are interrupted: “Okay students. Take a look at your math worksheet. Note there are addition and subtraction questions. Make sure you don’t use the wrong sign and remember I want to see how you solve the problems”. “Children in a band, please note that band practice will be in a gym at 12 noon”. “Oh and before you start, remember to put your name and the date on the top of your worksheet”. This is an illustration of working memory in a typically developing child. As the first instruction is given the student retrieves relevant information from long-term memory to hold online in working memory. Now the student interprets and generates internal representation of the teachers rather complex instructions and holds those online. With the PA announcement the student extracts critical information and stores it in long-term memory for later use and protects the existing information in working memory from interference. This is an illustration of working memory in a student with ADHD. First it’s important to remember that the student with ADHD may have a smaller working memory capacity to begin with. First, the student’s effective response to the history of repeated difficulties with math takes up valuable space in working memory. His slower processing speed impedes the translation of complex instruction, so he tries to hold them on line verbatim. But the PA announcement has tipped the scales. He’s on cognitive overload. Unable to block this irrelevant information which triggers retrieval of other information – where’s my drum? – and relevant information is lost from working memory. Moreover, there’s no room for those final instructions about details about name and date. So he is left with the Gestalt of the task – no details – with irrelevant information online that could disturb his performance.
A medicine widely used to treat attention-deficit/hyperactivity disorder also provides long-term relief from the attention and behavior changes that affect many childhood cancer survivors, according to a trial led by St. Jude Children's Research Hospital. Researchers reported that one year after starting the drug methylphenidate, young cancer survivors scored better on tests of sustained attention and other measures of attention, social skills and behavior than did a similar group of unmedicated survivors. Heather Conklin, Ph.D., of the St. Jude Psychology department, is the first author of the research published in the Journal of Clinical Oncology.
It seems like a paradox: we prescribe stimulants for kids who are hyperactive. Once we understand the neurobiology of ADHD, this approach makes a lot more sense.

Brendan Montano M.D., Primary Care Provider (PCP). Three scenarios are used to describe the signs and symptoms of Adult ADHD in patients. These include self-referred patients, patients who present a chaotic life and/or patients with comorbidities (anxiety disorders and depression) that may mask or emerge from ADHD.
ADHD symptoms can often be recognized in adults once they are identified as a child. Eva O'Malley was one of the first to recognize her ADHD symptoms after her son was diagnosed. Eva discusses indicators such as memory problems, inability to retain verbal information and her history interrupting others.
Adult Attention Deficit Hyperactivity Disorder can also affect performance at work and cause stress in personal relationships. Adults with ADHD are at risk of losing their jobs and experiencing marital problems. Doctor Montano urges primary care providers and psychiatrists to investigate if a patient describes their lives as chaotic or hectic. These issues could be related to attention deficit disorder. Two of Robert Whitaker’s books on this subject are among the most cited in the literature: Mad in America (2002) and Anatomy of an Epidemic (2010). He has received numerous awards (listed below), including a George Polk Award for medical writing and a National Association of Science Writers Award for best magazine article. He was a finalist for a Pulitzer Prize in 1998.
Whitaker’s work has alerted both the medical community and the general public to the serious dangers of psychotropic medicines when used for extended periods. He has become a standard reference for the facts.
Robert Whitaker is a prominent medical writer, whose concentration is on psychiatric care and psychotropic drugs. In this presentation he provides important information about how ADHD —now an “epidemic”— is diagnosed and why users of medications for ADHD should be cautious. Recent news stories about the hazards of long-term ADHD medications and the more general use of these drugs to enhance concentration underscore the importance of Whitaker’s discussion.
The American Academy of Pediatrics recently updated its guidelines for the diagnosis and treatment of ADHD. Tim Verduin PhD, Clinical Director at the Institute for ADHD & Behavior Disorders at NYU Langone Children Study Center, discusses why it is recommended that children younger than 5 years old start treatment with behavior therapy. He also discusses the reasons why, at any age of children, the AAP recommends that medication and behavior therapy be used together.
