A few days ago, I decided to set to private the few videos I've made to date on the subject of preparing Vyvanse water. Because I received a lot of comments about them, and didn't want it to disappoint anyone trying the drug for the very first time, I decided to make the videos private.

Instead of writing a video, I decided to make a video explaining (from the perspective of a patient) why the ADHD vs. Depression diagnostic process can be difficult for clinicians. After trying many different drugs, it took me more than a year to finally find the one that I believe and trust.

I have taken Wellbutrin and psychostimulants, so I wanted to share my thoughts on the differences. Or, rather, how they felt to my.

It is available in capsules, modified release tablets, and tablets.

How do you use it?
- Tablet: Take half of the tablet and drink half of it. You can chew the tablet or break it.
Modified release tablets or capsules: Take it with water. But don't break the tablet. This could reduce its effectiveness.
It is also a good idea to take Methylphenidate with a meal. This can reduce your chance of side effects.

When should it be used?
Methylphenidate stimulates the central nervous system. It is similar to amphetamines. It can be used for ADHD and narcolepsy, as well as during palliative sedation.

How do you determine the right dosage?
The exact dose you need will depend on the indication you are using, side effects you may experience, severity of your symptoms, and brand of methylphenidate that you use.

ADHD symptoms can be treated with regular tablets for children over 6 years old or adults. The usual starting dose is 5mg 2x/day. This can be gradually increased to 60mg/day if necessary.
It is important to know that modified release capsules and tablets may have different dosages.
Your doctor may prescribe 10mg/ 2-3 daily for adults with narcolepsy.

What are the most common side effects?
- Common (10%): symptoms include: insomnia, anxiety, headaches, palpitations and nausea.
- Common (1 -10%): upper tract infection): Shortness of breath, increased appetite, weight loss and aggression.
Uncommon (0.1-1%) Hypersensitivity reactions, psychotic disorders and suicidal thoughts, hallucinations.
Rare (0. 01-0.1%): mania, libido disorder, disorientation, chest pain, visual impairment and menstrual disorder.

Safety:
Be careful not to mix methylphenidate and alcohol. This could increase the drowsiness of methylphenidate.
It can also be dangerous to drive after taking this drug. It can make you sleepy, dizzy, or drowsy. Please don't drive if you experience any of these side effects. This is especially true during the first few weeks of using it.
You should also be aware that methylphenidate may interact with other medications you are currently taking. Always discuss these matters with your doctor.
The effects of methylphenidate on pregnancy and breastfeeding have not been properly studied. It could prove to be very dangerous, so it should be avoided. Discuss this with your doctor as there may be exceptions.

Do you think you might have a bipolar disorder? Eric Tivers and Malaika Tolford, the host of ADHD reWired, discuss Malaika's diagnosis of bipolar disorder. They also discuss how common it can be to have both ADHD and bipolar I.

Malaika is a ceramic artist, owner of Place Value Pottery, and teaches at a non-profit craft organization. They diagnosed Malaika with bipolar two disorder in 2013 and added an ADHD diagnosis in January 2020, which provided some pieces of her mental health puzzle. She is a mother to two boys and is trying to navigate COVID, while working remotely and staying out of the way her husband's zoom meetings.

Malaika talks about why she contacted Eric and the differences between bipolar I/II. She also discusses what led her to seek treatment and diagnosis for bipolar disorder. Malaika was suicidal and sought treatment for bipolar depression. The first thing she tried actually worked.

Malaika talks about what led her to believe she may have ADHD. She also discusses her struggle to discover who she was. Eric believes medication is the best treatment for ADHD if you are able to tolerate it.

Malaika talks about her pottery business and how it has evolved to online sales following the pandemic. She also discusses how pottery can be relatable to her diagnosis. This episode I discuss medical research on psychedelic compounds with Dr. Matthew Johnson, Professor of Psychiatry and Behavioral Sciences at Johns Hopkins School of Medicine. We discuss the biology of MDMA, LSD, psilocybin and ayahuasca. Attention deficit hyperactivity disorder, ADHD, is a common psychiatric condition affecting children's ability to function. As the name suggests, ADHD is characterized by difficulty paying attention and hyperactive or impulsive behavior.
There are 3 subtypes of ADHD:
- Predominantly inattentive,
- Predominantly hyperactive or impulsive,
- and the combined type with a mix of inattentive and hyperactive/impulsive symptoms.
Children with ADHD usually perform poorly at school, have difficulty with social interactions, and are often stigmatized.
Some children outgrow ADHD with age, while others continue to have symptoms into adulthood.
The diagnosis of ADHD is difficult and controversial. The key is to differentiate between normal developmental behavior and ADHD. Healthy children can SOMETIMES be inattentive, hyperactive or impulsive. It is totally normal for kids to have shorter attention spans and higher activity levels than adults or some of their peers. To be diagnosed with ADHD, symptoms should develop before the age of 12, persist for at least six months, and negatively impact daily activities in more than one environment, such as at school AND at home. Children who have problems at school but not at home, or vice versa, are likely suffering from something else other than ADHD.
To be considered as having ADHD symptoms, patients must experience AT LEAST SIX of the following:
For inattention:
- Failure to pay attention to details, making careless mistakes
- Having trouble staying focused in tasks or play
- Not finishing tasks
- Avoiding tasks that require mental effort
- Not listening when spoken to directly
- Having difficulty organizing things
- Losing things
- Being forgetful
- and being easily distracted
For hyperactivity/impulsivity:
- Fidgeting
- Being in constant motion
- Having difficulty staying seated when required
- Climbing on things in inappropriate situations
- Having trouble playing or doing things quietly
- Talking loudly and too much
- Talking out of turn
- Having difficulty waiting for their turn
- Interrupting or intruding on other people’s activities
Diagnosis is clinical, based on patient history, via interviews with parents, relatives and teachers. Physical exams are performed to rule out other conditions that may produce similar symptoms.
ADHD has a significant genetic component. A child has twice the risk of developing ADHD if his or her sibling has it. The condition is associated with dysregulation of several neurotransmitters, including decreased dopaminergic receptors in the frontal lobe.
Environmental risk factors include exposure to toxins, viral infections, nutrition deficiency, premature birth, and maternal drug use, alcohol use, or smoking during pregnancy.
Treatments can help improve symptoms and reduce the chance that symptoms persist into adulthood. Treatments include medications - stimulants and non-stimulants, and psychosocial therapies.
Stimulants are effective in about two thirds of patients and are considered first line therapy for ADHD.
Stimulants include amphetamines and methylphenidates. They block the reuptake of dopamine, thereby prolonging dopamine stimulation. Amphetamines also directly release dopamine.
Non-stimulants include antidepressants and alpha agonists. They are less effective and are usually prescribed only for patients who cannot tolerate stimulants.
All medications have significant side effects that must be closely monitored. It may take some time to determine the right medication for each patient and the lowest effective dose.
Psychosocial therapies include psycho-education for the family and psychotherapy for the patient. They can be effective when used in combination with medications. |

The truth about ADHD Masking.
People with ADHD don't always feel safe around others who don't have the disorder. We may feel judged and stigmatized by society, and as a coping mechanism, we hide or mask our ADHD symptoms to avoid feeling seen. 
Masking doesn't always develop in adults. Sometimes it's learned during childhood. Parents and other grown-ups would sometimes tell us to: « stop fidgeting », « stop talking so much », and "pay attention." While they mean well, children with ADHD symptoms learn that being themselves isn't safe, so they adopt a mask as a survival tool.
Another reason for masking is that most people with ADHD are misunderstood. We can't always comfortably be ourselves around others because we may be labeled rude or weird. And what can ADHD masking look like in different situations? Here are some common examples.
There are times when someone is speaking, and we listen or focus too hard on their words, so we don't miss any details.
Hyperactivity is sometimes masked by trying to feign calmness. But in reality, our mind jumps from one thing to another, and we can't correctly process what's happening at the moment. At social gatherings, we may try to copy other people's behavior to fit in. This can mean suppressing leg bouncing or simply nodding at someone talking, so we don't interrupt. Masking our symptoms can be exhausting, and truthfully, its disadvantages far outweigh its benefits.

One of the pitfalls of masking is that it can end up delaying our ADHD diagnosis. We hide it so well that we may not even recognize that there's already a problem. Remember getting an official diagnosis can help get us a proper treatment plan from a psychologist.
Another risk of masking is developing anxiety and depression. Because we bottle up so much stress without expressing it, the feelings might surface as fear, worry, loneliness, or even anger.
Lastly, we're not able to live as our genuine selves and express what we truly feel. We may feel that we're becoming someone else entirely because of our mask.

Adult attention-deficit/hyperactivity disorder (ADHD) is a mental health disorder that includes a combination of persistent problems, such as difficulty paying attention, hyperactivity and impulsive behavior. Adult ADHD can lead to unstable relationships, poor work or school performance, low self-esteem, and other problems.

Though it's called adult ADHD, symptoms start in early childhood and continue into adulthood. In some cases, ADHD is not recognized or diagnosed until the person is an adult. Adult ADHD symptoms may not be as clear as ADHD symptoms in children. In adults, hyperactivity may decrease, but struggles with impulsiveness, restlessness and difficulty paying attention may continue.

Treatment for adult ADHD is similar to treatment for childhood ADHD. Adult ADHD treatment includes medications, psychological counseling (psychotherapy) and treatment for any mental health conditions that occur along with ADHD.

Some people with ADHD have fewer symptoms as they age, but some adults continue to have major symptoms that interfere with daily functioning. In adults, the main features of ADHD may include difficulty paying attention, impulsiveness and restlessness. Symptoms can range from mild to severe.

Many adults with ADHD aren't aware they have it — they just know that everyday tasks can be a challenge. Adults with ADHD may find it difficult to focus and prioritize, leading to missed deadlines and forgotten meetings or social plans. The inability to control impulses can range from impatience waiting in line or driving in traffic to mood swings and outbursts of anger.

Adult ADHD symptoms may include:

Impulsiveness
Disorganization and problems prioritizing
Poor time management skills
Problems focusing on a task
Trouble multitasking
Excessive activity or restlessness
Poor planning
Low frustration tolerance
Frequent mood swings
Problems following through and completing tasks
Hot temper
Trouble coping with stress

Working memory is often referred to as an active mental workspace. It's a cognitive system that allows us to hold and manipulate information online for a few seconds during problem-solving or other cognitive activities. And it's his internal representation in working memory that we use to guide our decision-making, so that behaviour and actions are not dominated by immediate
sensory cues in the environment. By contrast, long-term memory is often referred to as a filing system for a more stable or longer-term storage of information. And typically cues in the environment or in working memory trigger the need to rapidly retrieve information from long-term memory and bring it into working memory along with other types of information.
Conversely, information in working memory may be reorganized and encoded back into long-term memory. Here's what might happen when a teacher gives instructions out of order and they are interrupted:
"Okay students. Take a look at your math worksheet. Note there are addition and subtraction questions. Make sure you don't use the wrong sign and remember I want to see how you solve the problems". "Children in a band, please note that band practice will be in a gym at 12 noon". "Oh and before you start, remember to put your name and the date on the top of your worksheet". This is an illustration of working memory in a typically developing child. As the first instruction is given the student retrieves relevant information from long-term memory to hold online in working memory. Now the student interprets and generates internal representation of the teachers rather complex instructions and holds those online. With the PA announcement the student extracts critical information and stores it in long-term memory for later use and protects the existing information in working memory from interference.
This is an illustration of working memory in a student with ADHD. First it's important to remember that the student with ADHD may have a smaller working memory capacity to begin with. First, the student's effective response to the history of repeated difficulties with math takes up valuable space in working memory. His slower processing speed impedes the translation of complex instruction, so he tries to hold them on line verbatim. But the PA announcement has tipped the scales. He's on cognitive overload. Unable to block this irrelevant information which triggers retrieval of other information - where's my drum? - and relevant information is lost from working memory. Moreover, there's no room for those final instructions about details about name and date. So he is left with the Gestalt of the task - no details - with irrelevant information online that could disturb his performance. |

Controversy sells, and the media knows it. So a lot of what we see, read about, and hear about ADHD and ADHD treatment either misrepresents the facts or is flat-out meant to scare us.

Netflix's new documentary "Take Your Pills" (interview w/producers: is no exception. This is why I'm upset: Despite bringing up many legitimate concerns, the film contributes to the confusion, stigma and fear around stimulant medication — many of the experts included either don’t have a background in research, directly benefit from spreading fear & misinformation about ADHD treatment, or speak on subjects outside their area of expertise.

This is a problem, because — as this study found ( — “Inaccurate and sensationalized media reports surrounding ADHD and its treatments [have] a major impact on parental decision making” when it comes to treatment.

Fortunately, a TON of research has been done on stimulant medications, so you and your doctor can decide what’s right for you or your child based on facts, not fear.

How often do you feel anxiety over time? Do you feel pressured and rushed, anxious because there’s never enough time or because time seems to be flying by too quickly? If you're watching this video, it's most likely because you're feeling a little stressed out due to something called time anxiety.

These are vicious cycles of thought which create a scarcity mindset where your thoughts continue to make you feel like you're worthless, not good enough, and that you're always going to be stuck like this and things won't ever improve.

Time anxiety can be one of the following or a mixture of both:
- You feel that you're behind in life.
- This is usually to do with feeling that you haven't achieved enough by a certain age and that you have wasted time in the past

- You feel that there isn't enough time to reach your goals anymore or to make up for the time you have wasted.
- This can lead to you feeling hopeless and overwhelmed causes you to start to panic over how much time you have left and that it's probably too late to do the things you want to do.
