Treating ADHD: Mom Gives 7-Year-Old Son Coffee Daily

When Christie Haskell saw the classic symptoms of Attention deficit hyperactivity disorder (ADHD) in her 7-year-old son, Rowan, she became concerned.

“At home there was a lot of just hyperactivity,” she told ABC News. “Not being able to his hands to himself, talking when he’s not supposed to talk, lack of concentration or ability to concentrate when he needed to.”

Convinced Rowan suffered from ADHD, but without an official diagnosis, Haskell turned to the Internet in search of a treatment that would ease her son’s attention difficulties.

Neurofeedback started as early as the 1960’s when researcher Joe Kamiya experimented with the Alpha activity (8-12 Hz) in the human brain. The results, which were very promising, inspired many other new experiments. Nowadays the fact that humans can learn to alter their brain function, by using an EEG, is virtually undisputed.

Details Matter:
The Meds Don't Work Long Enough: The Duration of Effectiveness [DOE] is not adequate: Each stimulant drug, each family of stimulant drugs has its own expected DOE. This DOE is not discussed in the literature as a measuring tool, but it's common knowledge with any who watches carefully for drug action and duration throughout the day.

All stimulant medications have an expected, less than 24 hr duration. Duration is an obvious point but is overlooked so often as an essential tool. If meds last less than 24 hr, then how long should each last? Dialing in the particular duration is necessary to get the best action out of each specific med. [AMP=Amphetamine Family, MPH=Methylphenidate Family]

Vyvanse [AMP] and Daytrana [MPH patch] both win the DOE race with 12-14 hr expected as optimum.

Adderall XR [AMP] is next with 8 hr DOE -- sometimes can get 10, but often more DOE is too much - out the top of the Therapeutic Window: w

Concerta [MPH] and Focalin [MPH] both run 8 hr if dialed in effectively - some studies show longer, my experience: more than 10 hr with either of these is too much -- see other articles on the 'Top of the Window.'

Metadate CR [MPH] and Ritalin LA [MPH] are both right at 8 hr, rarely longer, with side effects, if pushing the dose higher

The Immediate Release Stimulants, the tablets with no extended time
expected, last only a portion of the day with Adderall IR [AMP]
[Immediate Release Tabs] lasting about max 5 hr, Ritalin IR [MPH] is 4-hour max duration.

Methylphenidate (MPH)
Mechanism of Action:
Non-competitively, Methylphenidate blocks the reuptake dopamine/noradrenaline into terminals by blocking dopamine transporter and noradrenalinetransporter (NAT). This increases levels of dopamine/noradrenaline within the synaptic cortex.

Amphetamine can be classified as a d and l-isomer.
The active isomer is the d-isomer.
Mechanisms of Action :
1. Competing with noradrenaline and dopamine at NAT, competitive inhibitor at DAT
2. VMAT
Competitive inhibitor3. Reversal of DAT.

Becca is earning her master's degree in Industrial-Organizational Psychology and hopes to use her knowledge in individual differences, such as ADHD in personality and training in Industrial-Organizational Psychology, to help organizations create inclusive work environments for employees.

Becca discredits the idea that people with ADHD and Autism aren't conscientious. She discusses her understanding of Autism, how it manifests for her, as well as how it has affected her life. Eric talks about the differences between Autism and Aspergers, and the arguments surrounding them.

Becca talks about ADHD and how conscientiousness and gender affect ADHD. She also discusses her withdrawal from people due to her daily struggles. This is what led her into seeking a diagnosis. She discusses her sensory challenges and the constant struggle to be on-time.

Dr. Fruitman is often asked, “How do you choose the right medication?” This is a look at the thought process behind selecting the right medication.

Adderall and Ritalin are both in the same stimulant group. The patient’s attitude and preference towards each drug is a key component in choosing the right drug. It won’t work if a patient doesn’t take their medication.

It is important to find out the patient’s past experience with each drug. Dr. Fruitman suggests that patients should start with a low dose and an extended release form. Adderall is best for motivation, Ritalin for over-stimulation.

Both these drugs have been proven to be effective in treating ADD/ADHD. When deciding on a treatment plan, the most important thing is to choose the right dosage.

Social anxiety disorder (SAD) affects many young adolescents. SAD sufferers have a constant fear of being rejected or judged by others. This fear can negatively impact their work and school lives. This video explains what SAD is, and how it feels for someone with SAD. It was created to help educate young adolescents about the disorder.

This video was created by McMaster students Nadia Butt and Nour-Eddin Garada in collaboration with McMaster's Demystifying Medical Program.
