HIMSS Interoperability Showcase: Complete Care for Special Needs Patients

Joey, a 12-year-old, was diagnosed with autism spectrum disorder and attention deficit hyperactivity disorder (ADHD) at 5 years old and asthma at 7 years old. Joey takes Vyvanse for ADHD and Albuterol to treat asthma. He also sees a psychiatrist biweekly. Joey is suffering from severe pain and hasn’t seen a dentist for over two years. Joey does not currently have any health insurance. He needs immediate medication assistance. Joey is having trouble at school and his parents are not sure of their rights and accommodations. They also struggle to handle the many appointments, medical records, and care plans offered by various professionals to Joey. His family is understandably overwhelmed and in need of support. This problem is made worse by disconnected care planning, care coordination lapses, logistical complexities in delivery of clinical or non-clinical services, as well as a lack information and insight at the point where care is delivered and received. Joey and his family will be able to receive the care and services they need. It is crucial that all of Joey’s providers coordinate and consolidate their data. Learn how seamless interactions can be made between Joey’s multidisciplinary care team. From intake, appointment, care plan management, patient participation, education and monitoring, to care plan management, education, and monitoring, The connected care platform allows the care team to automate care coordination, referrals, and connections with healthcare consumers.
